Meaning
A woman's lover on the side — a boyfriend or kept man outside of, or in addition to, her main relationship. To have a 'sweetheart' is to have an intimate partner who is not one's husband, and the sweetheart-man is the male one, often spoken of in the tangle of gossip around who is 'keeping' whom.
